Registration and Update Submittals
Registration Forms, license copies, and insurance information may now be accepted electronically.
As of May 1, 2008 we would prefer, but do not require these submittals to be made via e-mail.
Instructions and helpful hints.
The Registration Form must be signed and notorized, so it is the one paper that must be printed.
Exception: When the license holder has a digitally encrpted signature and uses it on the form.
Once the form is completed it may be scanned & e-mailed, or faxed, or mailed.
To send License copies via e-mail:
Licenses may be looked up online and printed to a file, as a file type such as .pdf.
Licenses may be scanned and saved as a .pdf, .tiff, .jpg, or locked .doc file.
To send Insurance Certificates via e-mail:
Insurance Certificates shoule be sent directly from the insurer via e-mail.
Insurance Certificates may be scanned and saved as a .pdf, .tiff, .jpg, or locked .doc file.
They may be "printed to file" as a file type such as .pdf.
Note Insurance Certificates need to list us as the Certificate Holder
